John
John was referred to ABICS through social services. John had a stroke over ten years ago, but was having difficulties coping. He was diagnosed as having speech difficulties and having right sided upper and lower limb weakness. He was having problems organising and planning he lived in a first floor flat which due to his mobility issues he had trouble accessing. He was having problems with his family as his speech made it difficult for him to communicate. This inability to communicate made it difficult for John to access the community.
John was assessed by our ABICS co-ordinator and a care plan was put in place helping John with his problems.
He was moved to more suitable premises.
A routine was set up to enable John to cope better with day to day activities.
He was supported in setting up his finances so many of his bills were paid by direct debit.
After 12 months John was doing most things for himself.
The amount of care John received care package was gradually reduced over a period of a few months and he is now living independently in the community without any support.
