Case Study One

We received a call to the information line (01743 365565) from a hospital social worker with regards to a patient on the stroke ward. They were making plans for her to go home and finding out what services were available to her for continued support and rehabilitation. We left an information pack with the nurses for the patient and her family. We discussed options with the hospital social worker and contacted the local stroke co-ordinator and made her aware of the patient and the family so she could support them too.

The patient's husband made contact with us to discuss attending our Day Opportunities and so we arranged to meet with them and show them around to explain what services we offer here. Contact was then made with the social worker for them to proceed with their application for funding for the lady to attend. She started with us in our Day Opportunities 3 days a week. We support with speech and language, independent living skills and helping her adjust and know about her acquired brain injury.

Two months later, we contacted her husband and made arrangements to carry out a carers assessment to make sure that they had the correct support in place and if there was anything else missing or any other further support needed. From the assessment it became clear that the funding for his wife’s personal carer was not being recieved so we reported this to Social Services, as it was causing financial problems along with added stress. We suggested they contact CINCH for further support with this problem and gave them a leaflet on their services.

We now know that with the support of CINCH that the payments are now in place.

We continue to support the lady in out Day Opportunities 3 days a week and the family support for the other 4.

Carers Assessment

We are able to provide people who live within Shropshire Council's district with a Carers Assessment. We provide these on behalf of Shropshire Council to carers of people with an acquired brain injury. As we specialise in Acquired Brain Injury we are able to ensure that you get the specialist services you need. More information on Shropshire Council's Carer's Assessments can be found here.

Unfortunately we do not currently offer Carers Assessments on behalf of Telford and Wrekin Council but you can request one from them here.

Day Opportunities

Our Day Opportunities Centre is open Monday to Friday from 9:30am to 3:30pm. Our Outreach Worker, Claire, can come and visit you and if appropriate put the wheels in motion for you to attend our Day Opportunities. For more information and to watch a video, click here.
