Our Referral Process

- We’ll get in touch to answer any questions and set up an initial assessment.
- The initial assessment will help us understand your loved one’s needs, goals, and the best type of support we can give them.
- That’s it! The person you refer will start getting tailored support in their home, community, or at our Headway Hub.

Frequently Asked Questions
Do I need a formal referral to contact Headway Shropshire?
No. You can contact us yourself, a family member or carer can get in touch, or a professional can make or discuss a referral.
What if I do not know which service is right?
That is completely fine. Tell us what is happening and what support you are looking for. We can explore the Headway Hub, ABICS, Neuro Wellbeing Support Group, information or signposting with you and identify the best next step.
What information do you need when I first contact you?
At first we only need enough information to understand who needs support, how to contact them, what has happened, where they live, what support may be needed and who else is involved. We will ask for more detail only when it is needed.
What happens after a referral is received?
We will make contact within 24hours (Monday-Friday), understand the person’s needs, discuss the likely service and funding route, and explain the next step. You should always know what is happening next rather than being left wondering.
Can I contact you about someone without them knowing?
You can contact us if you are worried about someone, but we will need to consider their awareness, consent or other appropriate authority before progressing in the usual way. Safeguarding concerns are handled through the appropriate safeguarding process.
Do you need medical reports straight away?
No. We aim to ask only for the information needed at each stage and, where possible, use existing reports rather than making people repeat their story unnecessarily.
